Transformational Leadership in Higher Medical Education: Inspiring Curricular Change and Pedagogical Innovation in Health Professionals Training

Abstract

Introduction: Transformational leadership supports curricula change and pedagogical innovation at higher medical schools. In the current context, educational leadership in higher medical schools must cultivate cultures of continuous improvement, creativity and commitment.

Objective: To evaluate transformational leadership among administrators and faculty in higher medical schools for its strengthening in academic management oriented toward curricular change and pedagogical innovation.

Methods: This was a descriptive cross-sectional study with a mixed-methods approach, conducted during the 2024-2025 academic period among 85 administrators and faculty members from the Faculties of Medicine and Nursing at an Ecuadorian university of medical sciences. The sample was intentional and non-probabilistic. The Multifactor Leadership Questionnaire-5X (Spanish validated version), semi-structured interviews (n = 20), and review of strategic management plans were used. Descriptive and inferential statistics (means, correlations, SPSS software) and thematic content analysis were employed for the pedagogical proposals.

Results: Transformational leadership predominated (average 3.8 ± 0.6 in a 0-4 scale), with inspiration (4.1 ± 0.5) the strongest aspect and individual consideration (3.4 ± 0.7) the weakest. 68% saw a positive influence on pedagogical innovation and extra effort. Formatively, the categories of “need for leadership training” and “institutional barriers to curricula change” emerged.

Conclusions: Transformational leadership has a strong capacity to inspire in higher medical schools innovation and change for pedagogical innovation. It is proposed to strengthen training in transformational leadership in faculty development and institutional strategic plans.
